Filling out the Form

Print out this order form.
Fill in your purchaser information.
Fill in the "Quantity" for the title(s) you wish to purchase. Multiply the quantity by the price for each title, and enter the line total in the right column.
Sub-total all the right column line totals.
Enter the shipping & handling amount -- you can either multiply the sub-total by 0.15 OR use the shopping cart as if you were going to make a credit card purchase (you don't need a credit card to do the calculation) and use the amount that the shopping cart comes up with. If you are not in the US, please contact Gondola Webworks for assistance in calculating the correct shipping amount. When you get our reply email with the correct amount, simply enter that in the form.
Add the sub-total and the S&H amount to get the Order Total. This is the amount you need to make the cheque or money order out for.

Money orders are to be in US funds, and are to be international tender if originating from outside the US. Purchases paid for using US money orders are also shipped immediately.

If you prefer to pay using a personal cheque, shipment will be made only after the cheque is cleared. Cheques are to be in US funds and drawn on a US bank.
